This is an annual survey, so if you completed it last year, we hope you will participate again this year. Your responses will help the Oklahoma City Housing Authority make decisions about which services, processes, and buildings need to be improved. The survey is 11 questions long and should take less than 10 minutes to complete.

Your responses will remain confidential. This survey is being conducted by a professional survey company, ETC Institute, to ensure the confidentiality of the results. The survey is anonymous and does not ask for personal identifying information.

Get $10 for completing the survey. As our way of saying “Thank You” for your time, ETC Institute will email a $10 Visa gift card to you a few days after you complete the survey. You will see a PIN number at the top of the survey. This PIN number is only used by ETC to validate the survey and determine which households qualify to receive a gift card. If you choose to provide your name, contact information, and identifying PIN to ETC to receive the gift card, that identifying information will not be shared with the Oklahoma City Housing Authority.
